I posted this earlier in the Stellar Adventures forum, but I'm reposting it here in case anyone finds it useful.

This is the home-made fumble table I use in my AFF games. Some of the results are more story based, and not every result will work for every situation. But it often adds some memorable moments to a battle.

Roll two dice and read them as a two-digit number, 11 through 66.

ALTERNATIVE FUMBLE TABLE

11Your weapon breaks! (If magic, it’s now in the hands of the enemy.)
12Surprise blow! Take 1d3 damage from an unexpected source.
13Behind you! Foe’s next attack is at +3.
14Cornered! Foe’s next attack is at +2.
15Knocked out! Out of the battle for 1d3 rounds.
16Weapon knocked from your hand! Reduce your skill by two points. On your next successful attack, instead of harming your foe, you can retrieve your weapon.

21Knocked back! Over a ledge, into the water, into an unseen pit, etc. (If that's impossible, you get knocked into a wall, tree, etc.) Test Your Luck to catch yourself, or lose one die of Stamina on impact.
22Foe strikes a crippling blow! Normal damage, and you lose 1 Skill until healed.
23Stroke of bad luck! Lose 1 Luck.
24Unsteady footing! You slip, off-balance. -2 to rolls for one turn.
25Weapon breaks! Or runs out of ammo. (Enchanted weapon temporarily loses power.)
26Fall down! -3 to rolls while getting up next round.

31Foe strikes vicious blow! You take double damage.
32Foe stuns you! Take damage, -3 to your next roll.
33Smash! The nearest fragile object breaks.
34New scenery obstacle appears! -2 to rolls unless removed.
35Ground/floor gives away! You drop into whatever lies below. Test Your Luck to catch yourself, or lose one die of Stamina on impact.
36Errant strike! Hit the nearest ally (if no allies, hit self) for normal damage.

41Collateral damage!. You accidentally damage the most valuable object nearby.
42Something catches on fire!
43An item of your equipment is destroyed or lost.
44A piece of armor is damaged or destroyed (lose 1 point across).
45The situation suddenly gets worse: darkness, rain, rockslide, slippery floor, etc. -2 to rolls until fixed.
46Enemy reinforcements! One or two fresh new foes appear.

51Foe enraged with bloodlust! Regains 1d3 Stamina.
52Foe rallies! With a surge of morale, foe hits you twice. (Roll separately.)
53Merely stunned! Previously defeated foe rises with 1d6 new Stamina and attacks you.
54Alarm! An enemy sounds the alarm, which will soon summon reinforcements. Up to 3 more will arrive in 1d6 turns.
55It’s YOU! You recognize this foe as an old enemy: add 1d3 to enemy’s Skill and Stamina.
56Look out! Foe unleashes an unexpected new ability, enchanted artifact, or spell.

61Foe gets lucky strike, hits both you and nearest ally.
62Foe strikes weak spot. No armor roll (if no armor, +2 to damage roll)
63The battle disturbs a random unrelated creature/foe, which immediately attacks you.
64Driven back! You may only defend next round, not attack.
65Lose your grip. Weapon goes flying, cannot be retrieved until after the battle.
66Lucky accident. Miss intended target, but hit a random other foe for normal damage.
